WAIVER AND RELEASE OF LIABILITY

Organization Name: The Holistic Ann’Venture (Anne-Laure Herrera)

In consideration of being allowed to participate in the Holistic Ann’Venture offered by the Coach Anne-Laure HERRERA, and the use of her equipment. I the undersigned, acknowledge, appreciate, and agree that:

1. It’s my responsibility to consult with a physician prior to and regarding my participating in any activities from The Holistic Ann’Venture. I represent and warrant that I’m physically fit and I have no medical condition that would prevent my full participation in the training. If I do have a health condition I will notify Anne-Laure HERRERA the coach BEFORE to start any session.
2. In consideration of being permitted to participate in any Ann’Venture activities I FREELY AGREE to assume full responsibility for any risks, damages or injuries, including death , both known and unknown, which I might incur as a result of participating in any actvities.
3. I KNOWINGLY, VOLUNTARILY, and EXPRESSLY waive any claim I may have against the coach Anne-Laure HERRERA for injuries or damages that I may sustain as a result of participating in any activities.
4. I, my heirs or legal representatives forever release, waive, discharge, and covenant not to sue Anne-Laure HERRERA for any injuries, including death, or damages caused during any activities.
5. I agree that during any activity, I will not engage in behaviour deemed inappropriate, including, but not limited to, vulgar language, bullying, harassment, intimidation, violence, discrimination, and that behaviour deemed inappropriate will result in the immediate termination of my participation, without refund, and I will be asked to leave the premises immediately.
6. I’m aware and fully accept all known and unknown risks, including the exposure to respiratory illnesses such as COVID-19. I understand to follow all AHS guidelines. I understand that I can be expose to the coronavirus to its symptoms through no one faults. I agree to hold Anne-Laure HERRERA harmless and I will not hold her or any other members liable for any symptoms of COVID-19.
7. I fully agree to accept all risks of entering the facility, using equipment, working with a trainer, attending classes/bootcamps and or being exposed to other members.
8. By agreeing to this document I give Anne-Laure Herrera to use any and all pictures or video of myself taken during any Ann'Venture programs. I understand that it will be used for The Holistic Ann’Venture publications or exhibits including the website and social media. If I do not want my photos or videos to be used I will contact Anne directly to let her know.
